Program Description

Home Addiction Recovery Treatment (H.A.R.T.) is a specialized program designed for individuals facing substance use challenges who prefer or require care at home. Whether due to barriers accessing traditional rehab or a desire for greater privacy, H.A.R.T. delivers fully coordinated, integrated, and individualized care — maintaining the highest standards of safety, professionalism, and compassion.

Participation in H.A.R.T. is determined through clinical screening, which evaluates each client’s:

  • Level of risk
  • Motivation for recovery
  • Home environment stability
  • Ability to comply with treatment recommendations

Treatment coordination is tailored to the individual’s acuity. All services are delivered in the client’s home environment under strict clinical protocols and eligibility guidelines. In-home treatment requires that clients present low to no physical or behavioral risk to the clinicians providing care.

H.A.R.T.’s in-home program may be right for you if you:

With H.A.R.T., you’ll receive support from a licensed team of certified addiction professionals, therapists, and medical experts — all working together to support every part of your recovery: clinical, emotional, medical, and family-based, delivered directly to your door.

 

  • Medical Director – Oversees in-home withdrawal management protocols and supervises medical staff
  • Nurses – Conduct medical assessments, monitor detox, and ensure your health and safety at home
  • Clinical Director – Oversees therapists, case managers, and ensures quality clinical standards across all programs

Preliminary Screening

We assess basic eligibility, including level of risk, home environment, and motivation.

Comprehensive Assessment

A licensed clinician conducts an in-depth biopsychosocial evaluation to identify needs and develop a tentative care plan.

Approval and Scheduling

Once cleared, we establish your care team, schedule your first visits, and initiate onboarding.

Start of Services

Within days, our medical and clinical team arrives at your home to begin your treatment journey.
